Conducts accurate comprehensive assessment of individuals and groups presenting without appointments to improve outcomes and minimise adverse events;
Assesses and develops health care plans that direct health care interventions and activities by others;
Provides care in the general practice environment as well as in homes and other community settings;
Identifies and provides comprehensive physical, psychological and emotional care for individuals and groups;
Conducts health care clinics;
Provides health care services in accordance with Medicare Benefits Schedule conditions;
Assists with minor surgical procedures undertaken by the general practitioner.
Undertakes health screening and health monitoring activities such as cardiac assessment including electrocardiographs and stress tests;
Involved in chronic disease management activities such as preparing a management plans, coordinating care, providing education about self-help strategies;
Provides timely and accurate information and education throughout the practice
Undertakes care planning, CVCs and reviews;
Undertakes various health assessments including (by not limited to) – over 75 years annual assessment, 4 year old checks, 45-49 assessment, 40-49 year old diabetes at risk assessment,
Conducts immunisation, wound care and chronic disease management clinics.
Ensures that recall registers, pathology systems and individual and group documentation systems are continuously reviewed to achieve optimal outcomes; working closely with the administration team to coordinate the recalls/reminders systems
Collaborates with individuals, groups, general practitioners, other general practice staff and health care providers in the development and review of guidelines, protocols or templates;
Monitors infection control processes in the general practice including compliance with sterilisation guidelines;
Participates in practice accreditation processes.
Ensures that process used for cold chain management complies with the current edition of the National Vaccine Storage Guidelines
Registered nurse licensed to practice in NSW and drivers licence;
Experience working in a primary health care setting;
Experience working with older people and people with chronic illnesses
Able to work as part of a friendly team
Excellent communications skills with patients, their families and with other members of the general practice team. Working together with the practice teams, is an important part of your role.

We are a eight doctor practice, with a great team of friendly health professionals, and a skilled clinical and administration team. Culture and a good working enviroment is held highly by the Practice Manager. It is important to enjoy coming to work and feeling supported by the team around you. The practice is currently being renovated to expand to ten consultation rooms, with a newly four bed treatment room on the plans for 2020. You will be working in a newly renovated treatment room. We are centrally located in Killarney Vale on the Central Coast of NSW, with plenty of parking one hour from Hornsby. We currently have two part time registered nurses, and we are looking for a nurse that will predominently fill about 35 hours per week.
Some flexibility with rostered hours and days, and school children. Permanent Part time position, with permanent rostered hours. New treatment room, and newly renovated practice. Regular clinical and administration meetings that focuses on good communication and colaborating with both clinical and administration teams.
